The procedure should be predictably reproducible, without causing functional loss in the donor area, and leave the patient with minimal deformity and scars.
Protecting the Testes: In scrotal reconstruction, a major concern is the protection of the testes to prevent damage to sperm production. Testicular insult can be minimized initially using wet dressings or temporarily positioning the testes in subcutaneous thigh pouches.
Preserving Erectile Function: In penile skin reconstruction, preserving the erectile function of the penis is crucial. Any reconstruction method should not impede sexual intercourse.
Although thigh pouches may offer a short-term solution for testicular positioning, long-term use can lead to pain, psychological issues, and significant abnormalities in sperm count after two years. Therefore, thigh pouches are not recommended as a permanent solution for patients wishing to preserve fertility.
Local flaps offer several advantages over grafts for testicular and penile skin coverage. These advantages include a reduced risk of complications such as skin maceration, poor graft take, and graft breakdown due to fecal and urinary contamination. Additionally, local flaps require less intensive postoperative care.
Various muscle skin and skin flaps derived from the perineum, groin, pubic region, and thigh tissues can be employed for scrotal and penile skin reconstruction. Some commonly used flaps include the medial circumflex femoral artery perforator flap, the gracilis fasciocutaneous flap, the neurovascular pedicle pudendal thigh flap, and the Singapore flap.
