Facial scars resulting from burns can have a profound impact on a patient’s social and psychological well-being in addition to the physical deformity. These scars can lead to difficulties in speech and eating due to stretched lips and chin. Eyelid contractures, when present, can hinder the ability to close the eyes fully, potentially irritating and, in the long term, leading to corneal damage and even blindness. Nose burns may result in airway obstruction and breathing difficulties.
Scarring on the face can result in significant cosmetic deformities that affect daily life, leading to lowered confidence and self-esteem. Loss of eyebrows, ears, and the presence of scar bands on the face can cause individuals to hide their faces even from their family members. Addressing these issues requires early and adequate treatment to improve the patient’s quality of life.
In cases of major facial burns, a single procedure is often insufficient, and multiple stages of procedures may be required for complete facial reconstruction. The choice of treatment depends on the severity and location of the condition. Common treatments include Z-plasty, skin grafting, or the use of local flaps from adjacent healthy skin.
Priority is given to addressing eyelid contractures, which should be treated urgently. This involves releasing the scars over the eyelid region and covering the area with a skin graft.
Large, scarred areas may necessitate the replacement of scarred skin with adjacent normal skin, and this is where tissue expanders play a crucial role. Tissue expanders are inserted into adjacent healthy skin and gradually expanded, resulting in a significant improvement in both appearance and function.
Stretching of the lips can be corrected through release and skin grafting. Ear reconstruction can be achieved using a prosthetic ear or carved from one’s rib cartilage. Hair transplantation is an option for those who have lost their eyebrows. Fat grafting can restore lost volume in the face due to burns, and nanofat can be used to enhance skin quality.
While it may be challenging to provide a completely normal physical appearance in severe burn cases, the goal is to optimize functional outcomes to enhance the patient’s daily life.
The patient is typically admitted a day before the surgery, and routine pre-surgery investigations are conducted. During the surgery, scars are excised as needed, and the resulting raw area is covered with a skin graft or a flap from an adjacent area. In cases where tissue expanders are used, the procedure involves two stages.
First, an appropriately sized tissue expander is inserted into adjacent healthy skin, which is gradually expanded on a daycare basis after the initial surgery.
Once the expander has sufficiently expanded the overlying skin, a second-stage surgery is performed, and the expander is removed. The extra skin is then used to cover the defect created after scar excision.
Following the surgery, the patient is typically kept in the hospital for 2 to 3 days. The first dressing is usually done after 7 days, and regular follow-ups are conducted weekly. Once the area has completely healed, preventive measures should be taken to minimize scarring on the face.
These measures include regular massage with coconut oil, silicone gel, and sheets, as well as sun avoidance.
