Blast Injuries: These injuries occur due to explosive devices, leading to complex polytrauma cases where the treatment of other injuries takes precedence over the hand injury. In cases of active bleeding, hemostasis is a top priority, alongside managing other injuries.
Cracker Burn Injuries: Cracker-related injuries to the hand can be of two primary types:
Flame Burns: These injuries are typically managed with routine dressings, antibiotics, and pain relief medications.
Blast Injuries: Occur when a cracker explodes while held in the hand, resulting in surgical emergencies that require immediate attention.
Managing a mutilated hand due to blast injuries, considering factors like limb ischemia and amputation, is a complex process that demands specialized skills and expertise. If there’s limb ischemia or partial amputation, the possibility of reimplantation is considered. The amputated part should be carefully transported in a plastic bag with ice at an ideal temperature of 4 degrees Celsius. Once the patient’s overall condition stabilizes and other critical injuries are addressed, the hand injury is evaluated.
Patients and their families receive detailed counseling regarding the complex nature of managing blast injuries. It’s essential to clarify that these injuries typically require multiple stages of surgery and extensive effort from both the surgeon and the patient’s support system.
After initial management, the patient is planned for surgery for initial debridement to be done under suitable anesthesia.
Reimplantation is not possible or feasible in most of the cases of hand blast injuries even if the part is brought well preserved as there is injury to the vital structures at multiple levels within the part or the part is damaged beyond recognition or is in shreds. In a few isolated cases, where the part is well preserved relatively, reimplantation can be attempted with guarded consent.
Debridement of all necrotic and dead tissues + wound irrigation of wound culture
Antibiotics coverage with skeletal stabilization with K wires
In a few cases, a second relook surgery is required to debride any remaining dead tissue.
Skin and Soft Tissue Reconstruction
Once the infection load and edema decrease, the patient is planned for second-stage surgery.
Tendon, nerves, and muscles are repaired.
Appropriate soft tissue coverage is important for coverage of bone, joint, tendon, and neurovascular structures. The closure is done as per the protocol of the reconstruction of soft tissue defects of the hand.
The outcomes of surgery for blast injuries vary depending on the extent, type, and level of injury. The road to rehabilitation can be long and challenging, and results may differ from initial expectations. The primary goal is to reintegrate patients into society with a functional hand, although this may not always be achievable.
