Tissue expansion is a specialized procedure used to address wide and unsightly scars on the skin, particularly on the face, neck, and scalp. The technique involves the placement of an expandable balloon-like implant beneath the skin. It is referred to as “skin expansion by tissue expander,” as it generates additional skin through mechanical and biological overstretch.
Tissue expansion typically involves two surgical stages:
First Stage: During the initial surgery, a silicone balloon-like implant is placed under the normal skin adjacent to the scarred area. The implant is then gradually expanded at regular weekly intervals after the surgery until the desired amount of additional skin is generated.
Second Stage: In the subsequent surgical stage, the expander is removed, and the scarred region is excised. The expanded normal skin is then used to replace the scarred skin, resulting in improved aesthetics.
Tissue expansion is favored for scar revision and other applications due to several advantages:
Minimal Scarring: The procedure offers minimal scarring, as skin from an adjacent area is used, leading to a single suture line along the scar margins.
Excellent Match: The color, texture, and thickness of the skin closely match the surrounding area, ensuring a natural appearance.
Low Risk of Rejection: There is a minimal risk of rejection because the patient’s own tissue is used.

Tissue expanders are commonly used in the following scenarios:
Scar Revision: Removing wide scars on the face and neck region.
Post-Burn Hair Loss: Treating hair loss over the scalp after burns or addressing baldness by expanding adjacent hair-bearing skin.
Deformity Correction: Correcting various deformities on different parts of the body.
Breast Reconstruction: Placing expanders beneath the chest skin to prepare for breast reconstruction, allowing for the desired breast contour and size.
Microsurgery: Expanding skin on the back or buttocks for later transplantation to other body parts using microsurgical techniques.
The surgery at Kayakriti is a two-stage procedure.
In the first stage, an appropriately selected expander is inserted adjacent to the scar or defect.
The patient is typically discharged a day or two after this first surgery, with stitches removed around the 12th day.
After two weeks, the expander is gradually inflated with normal saline at weekly intervals.
Once the desired expansion is achieved, which usually takes 8-12 weeks, the patient is admitted for the second stage surgery. During this stage, the expander is removed, and the expanded skin is used according to the patient’s needs.
After the second procedure, patients are usually highly satisfied with the results. Regular follow-up appointments are scheduled to assess the proper maturation of the scar and ensure the patient’s well-being.
