Oral Submucous Fibrosis is a complex condition characterized by the significant stiffening of the mucosal lining inside the mouth, leading to a progressive inability to open the mouth. This condition is considered precancerous and is primarily caused by the consumption of tobacco.
OSMF typically begins in the posterior part of the mouth’s interior and spreads outward, potentially affecting the entire oral cavity. Since it is a precancerous condition, there is a risk of it transforming into oral cancer. OSMF develops in stages:
Inflammation: The condition starts with inflammation of the oral mucosa.
Fibrosis: This is followed by the development of fibrous tissue and blanching of the oral mucosa.
Trismus: In advanced stages, patients experience restricted mouth opening, a condition known as trismus, due to the formation of fibrous bands.
The primary cause of OSMF is the use of tobacco, whether in the form of chewable tobacco or through smoking. To prevent OSMF, the following steps are recommended:
Health education and awareness regarding the cessation of tobacco use.
Regular oral cancer screening by healthcare providers.
The treatment of OSMF depends on the stage at which it is detected:
Early Stage (Grade I & II): Medical management is recommended, including the use of anti-inflammatory, oxygen radical scavenging, and antifibrotic drugs. Corticosteroids may be used during the early inflammatory phase. Other treatments such as hyaluronidase, chymotrypsin, collagenase, pentoxifylline, iron, and lycopene have limited roles.
Advanced Stage (Grade III & IV): Surgical intervention is necessary. This may involve excising fibrous bands and using grafts or flaps to maintain increased mouth opening.
Various surgical procedures are performed based on the extent and severity of OSMF:
Primary Excision: Fibrous bands in the buccal mucosa, tongue, and soft palate are excised. This procedure is effective for less severe cases.
Conservative Adjunctive Surgical Procedures: These include detaching the coronoid process from muscle attachment (coronoid-ostomy) or intentionally cutting the coronoid process (coronoid-ectomy) when adequate mouth opening cannot be achieved with excision alone.
Interposition Grafts & Flaps: Grafts or flaps are used to maintain mouth opening after surgical excision of fibrous bands. These can include skin grafting, local flaps, and distant flaps.
Post-surgery care is crucial and includes extensive physiotherapy to maintain mouth opening achieved through surgery. Cessation of tobacco use is essential to prevent relapse. Prosthetic devices may also be used to guide healing tissues and stabilize grafts.
Physiotherapy with mouth openers, dietary supplementation, and regular monitoring of mouth opening are key components of post-operative care.
