The surgeries performed to rejuvenate the female external genitalia are called Vaginal/female external genital rejuvenation procedures. The female external genitalia/vulva are the accessory structures of the female reproductive system that are external to the vagina. Vulva consists of mainly Labia Majora, Mons Pubis, Labia Minora and the clitoris. The Labia Majora are large, fleshy folds of tissue that enclose and protect the other external genital organs. Labia minora are small fleshy folds lying inside the labia majora and surrounds the openings to the vagina and urethra.

The labia minora come together anteriorly and form a small bulge called clitoris which is extremely sensitive and plays avital role in achieving orgasm in women. The clitoral hood conceals the clitoris nestled inside, similar to the prepuce of penis where the foreskin sheathes the head in its non-erect state.

In some women, this clitoral hood is large and has extra tissue which can lead to irritation as it continuously rub against the clothes. Decrease in sensation of the clitoris is also seen in women due to large clitoral hood. A clitoral hood reduction surgery or clitoral hoodectomy, or clitoridotomy is a cosmetic surgical procedure which removes the excess skin (prepuce) that covers the clitoris. The technique not only enhances the visual appearance of a woman's pubic area, but it also serves to improve sexual pleasure by making the clitoris more accessible. This surgery is often referred as feminising genitoplasty.

Preparations to be done for the surgery

  1. If you are a smoker, you should stop smoking at least two weeks prior to the surgery.
  2. Get the tests done prescribed by Dr Amit Agarwal.
  3. Avoid taking anti-inflammatory drugs and herbal supplements as they aggravate bleeding.
  4. Admission will be done on the same day at the centre
  5. Written consent form will be signed explaining the procedure after the admission.
  6. Meeting with the anaesthetist for Pre-Anaesthetic check-up before the surgery if you are planning to undergo this procedure under general anaesthesia.
  7. Nil per oral for minimum of 5-6 hours before the planned procedure.

Clitoral hood reduction surgery procedure

  • Normally clitoral hood reduction is done as a day care procedure, though if you are not comfortable with local anaesthesia; it can be performed under general anaesthesia or Spinal Anaesthesia.
  • Next, an incision is made in the clitoral hood and the extra projection of the hood surrounding the clitoris and excess tissue is removed. Great care is taken in not to disturb the tissue and nerves of your clitoris. The incision is closed with dissolvable sutures.

Recovery after the surgery

After your clitoral hood reduction, you may experience some mild swelling and bruising which can last upto 6 weeks. You may also experience some discomfort. You will be made to walk on the same day of the surgery. You will be discharged on the same day of the surgery. The sutures will be dissolvable and do not require to be taken off.

To help with healing, take two to three days off from your daily routine. During that time, make sure to get plenty of rest and avoid straining yourself physically or emotionally. You can use ice packs every 30 mins if the swelling is more. Elevate your bottom with pillows when lying down at home. Gently pat dry the area after bathing and toilet and do not wipe or rub it. Always wear loose-fitting clothing to avoid irritating the area.

Patients can resume their normal routines 3-6 days after surgery and can have intercourse after 4 to 6 weeks after the surgery. You have to avoid any rigorous contact to the area for a minimum of one month. This includes sexual activity and horseback riding.

At first you may be a little numb but as the swelling subsides, sensation will return.

Risks and complications after the surgery

Like every surgery, clitoral hood reduction surgery also involves some risks and complications which include

  • Abnormal discharge
  • Rare chances of bleeding, infection and delayed wound healing(mostly seen in smokers, people with high BMI, diabetics, hypothyroidism and in women with multiple diseases)
  • Altered sensation which improves with time.
  • There can be pain which subsides with time and slight medication.
  • Prolonged swelling and bruising.
