What is Bedsore?

Bedsore is the wound being developed over the body due to lying for an excessiveamount of time in the same position on the bed. This happens when the body is quite sick and unable to move due to some primary ailments, like paralysis, trauma, fractures, old age, etc.

When do people get bedsore?
  • Patients admitted in the hospital for long time, especially in ICU or on ventilator, are prone for developing bed sores.It is observed that about 3-14% of hospitalised patient develop pressure sore.
  • Any prolonged ailment which makes the patient immobile and confined to bed or wheelchair; hence the prolonged excessive pressure over susceptible areas makes them prone for developing a bed sore.
Where do people get bed sore?

It is the position of the patient which decides the area of pressure sore, e.g.

The other areas which develop bed sores are heel, elbow, malleolus, knee, scapula and occiput.

Why do people get bed sore?

The reasons behind developing pressure ulcerare-

  • increased fragility of skin of the bed bound patient,
  • bad nutritional status compounded with
  • inadequate frequent change in position of the patients at home by the relatives or at hospital by the nursing staff.

What are the Risk factors for developing bedsores?

  • Extrinsic factors include pressure, shear, friction and moisture. These are external factors, if taken care of, there will never be a bedsore.
  • Intrinsic factors include the ongoing problems inside the body which makes the patient weak and less able enough to heal any wound present onthe body by itself. They can be sepsis(generalized infection in the body), decreased control over bowel and bladder, old age, diabetes, peripheral vascular disease, history of smoking and malnutrition. All the internal factors prevent protection against any external pressure or injury.

What are the different Categories of Bed Sore?

We often categorize the bed sores with respect to the amount of damage.

  • In category I, there will be generalized redness over the pressure areas.
  • In category II, there will be partial loss of skin, like we get injured while rubbing forcefully over any object. These two categories are still not worrisome and will heal once the pressure is off loaded and proper care of the wound.
  • In category III and IV, there is full thickness loss of skin and muscle, bones are exposed, respectively.

    These two categories if not taken proper care at the right time may risk life of the patient.

Therefore, whatever the stage maybe, relatives or attendants should consult the plastic surgeon immediately and start the management to stop further progression of the bed sore.

What is focus of Dr Amit Agarwalduring consultation?

Once the patient reaches the hospital, full investigations are done to know the primary ailment of patientand examination is done to know the exact stage of wound.

  • If any neurological problem is there, neuro-physician is consulted.
  • For diabetes management, physician is consulted for optimisation.
  • Nutrition is optimized, serum albumin level must be kept above 2.8 g/dL.
  • Pus discharge, if any, is sent for culture and sensitivity and antibiotics started accordingly.
  • Osteomyelitis, i.e. infection in bone, needs to be ruled out, so digital X-ray of the involved region is sometimes done.
  • Surgical management is needed if the bedsore is deep

What are the Goals of management for a patient with a pressure sore?

  • Prevention of complications due to existing sore
  • Preventing the existing wound from getting larger
  • Preventing sores in other locations
  • Closure of the wound.

What is the main advice and management for such patients with bedsore?

  • To maintain proper hygiene,
  • To keep the area moisture free.
  • To mobilize as early as possible and to change position as much as possible.
  • Side to side turn or front to back position change is advised every 1-2hourly, depending on the area involved.
  • Pressure should be off loaded from the area, and Air mattress is advised.
  • Nutrition and Medical management.
  • Surgical management is needed if the bedsore is deep

How is a bedsore treated at kayakriti?

A. For categories I and II bedsore

The patient might not need admission if other comorbidities and factors are under control. Attendants are advised all the management described above and may be sent home with oral antibiotics and regular specific dressings advised.

B. For categories III and IV bedsore

The patient definitely needs admission and all the investigations are done with respect to surgical correction. Once the patient is vitally stable and clear, surgical correction is planned according to the site of body involved.

Usually, three main areas are involved – SACRAL, ISCHIAL and TROCHANTERIC pressure sore.
