Preparations to be done for the surgery

  1. You should be physically healthy.
  2. You should stop smoking if you are a smoker before at least two weeks prior to the surgery.
  3. Get the tests done prescribed by Dr Amit Agarwal.
  4. Avoid taking anti-inflammatory drugs and herbal supplements as they aggravate bleeding.
  5. Admission will be done on the same day at the center.
  6. Written consent form will be signed explaining the procedure after the admission.
  7. Compression garment measurement should be given for post-surgical use
  8. Meeting with the anaesthetist for Pre-Anaesthesia Check-up (PAC) before the surgery. All pre surgical work up investigations should be shown to the operating surgeon and the anaesthetist, and we will evaluate your fitness for the surgery. You will be given date for the surgery only after you are declared fit for the procedure.
  9. Nil per oral for minimum of 5-6 hours before the planned procedure.
  10. Breast reduction is a surgical procedure so you should always have an attendant who can accompany you after the surgery for at least 24 hours and drive or take you home after the procedure

Breast Reduction or Reduction Mammaplasty procedure includes

The best and appropriate technique of breast reduction will be determined on the basis of following factors by Dr Amit Agarwal and you:

  • Breast size and shape
  • Amount of reduction required.
  • Elasticity as well as the amount of extra skin to be removed.
  • Degree of breast sagging.
  • Size and position of your areolas.
  • Combining additional procedure of breast lift if required.

Your condition, amount of reduction desired and your breast Composition and the surgeon's advice will decide which technique will used for the surgery.

In the first step, general anaesthesia is given by our very competent anaesthetist and intensivist.

In the second step, Breast Reduction Surgery is performed through incisions on your breast for the removal of fat, glandular tissues and skin. Incision option include:

  • A racquet shaped pattern or a keyhole incision is made around the are ola and moving vertically down to the breast crease.
  • Brow Forehead Lift

  • An inverted T or Anchor shaped incision pattern. This incision starts around the areola, moving down vertically to breast crease and then horizontally along the breast crease.

Scars will be very faintly visiblealong the incision linebut are well hidden under the swimsuit or bra.

Third step includes removal of the extra hypertrophied breast tissues with repositioning of the nipple- areola complexon both the sides. The size of the areola is reduced by excising skin at the perimeter. The underlying tissues of the breast are reduced, lifted and shaped accordingly and nipple is positioned to a new natural height. Liposuction of the lateral chest wall area is combined usually to give better definition to the area.

In the fourth step, the incisions are closed to reshape the smaller breasts. Suturing is layered deep to support the newly shaped breasts. After re-draping and removal of the excess tissues, the remaining skin is tightened, and the incisions are closed with the help of skin adhesives over drains. Drains are placed under the skin to remove excess blood or fluid from the surgery. Incision lines fade and improve over time but are permanent.

Scarless Breast Reduction with liposuction

Scarless breast reduction adopts liposuction technique for breast reduction. Since breast is composed of fat and gland elements, liposuction can suck the fat out and reduce the size of the breasts.

This unique technique of liposuction offers several advantages compared to conventional technique which requires long skin incisions on the breast region. These include less trauma to the breast, faster recovery, very few visible scars as the incisions are very small (with two on each breast, each about 3 mm in length), almost invisible to the eye once healed and fewer postoperative complications, improvement in shoulder and neck pain, and more natural appearing results with very less downtime.

Conventional procedure is still definitely the gold standard method for breast reduction for women with large sagging breasts but there are more scars, the surgery takes longer and there are sutures and drains involved. Also, more complications are possible with this type of breast reduction. Liposuction assisted scarless breast reduction will produce a smaller version of the breasts that a woman had before the procedure but liposuction cannot decrease the breast size markedly when compared to conventional technique. The shape of the breasts will be quite similar before the surgery but will be smaller in size and is ideal for women who want smaller, proportionate and youthful breasts, and do not desire or require significant reduction. Women with good skin tone, limited sagging, and an excess of fatty breast tissue, rather than glandular tissue, may be good candidates for scarless breast reduction. Women with moderate to severe sagging and with excess glandular tissues and less fat are not good candidates for this procedure.

Recovery after the surgery

After the surgery, dressings are applied to the surgical area, and you are made to wear a compression garment to reduce swelling and to support your Breasts. You will be discharged next day of the surgery after the drains are removed followed by the weekly visits or as suggested by Dr Amit Agarwal.We may send you home with drain if there is excess fluid in the body. It might be preferable to remove the drain at a follow up visit. The complete result of Breast reduction Surgery will be seen after the swelling and bruising subsides. You should give your body 3 - 6 months' time for evaluation of best results.

  1. There can be substantial swelling and bruising of the breast area which usually subsides in 1 to 2 weeks
  2. The sutures will be mostly within the breast crease, around nipple-areola complex which will be dissolvable and do not require to be taken off.
  3. In the days following surgery, there will be pain which can be subsided by the pain killers given by Dr Amit Agarwal.
  4. Soreness might last for few weeks.
  5. You will be made to walk 6 hours post-surgery or the very following day, depending on your will and wish.
  6. You can start exercising after three to four weeks post-surgery.
  7. Pressure garment is a must wear for the next 3 months without gap or fail for almost 24 hours a day.
  8. You can return to work depending on your will power and the type of work required between 6-10 days.
  9. You can return to your normal life style after two weeks of surgery. After two weeks you will be confident with your new up lifted profile.

Risk and complications post-surgery

The complications and aftermaths of the surgery vary from person to person and her physical well-being. The factors which affect are poor circulation, a history of blood clots, diabetes or heart, lung or liver infection.

  • Rare problems related to General anaesthesia.General Anaesthesia is relatively very safe in this era and time zone and the chances are very rare to land up in any complication. But no one can predict who will land up in complication.   • Risks related to higher BMI (Body Mass Index).Higher the BMI, higher are the risks of wound healing problems
  • Seroma formation or fluid accumulation below the breast tissue can occur which subsides with time.
  • Rare chances of infection and wound dehiscence are there which may require prolonged antibiotic therapy and re-suturing respectively.
  • Bleeding or haematomaformation under breast skin flaps which might mere require observation or surgical drainage.
  • Poor wound healing due to poor blood supply, seen more in smokers, people with medical conditions like diabetes, hypothyroidism or multiple diseases. Poor blood supply can lead to skin discoloration to skin/nipple-areola necrosis (rare).
  • Numbness or altered skin or Nipple sensation.
  • Inability to breast feed.
  • There will be faintly visible scars present along the incision which might hypertrophy only if you are prone for it.
  • Persistent pain which subsides with time and mild to moderate medications.
  • Deep vein thrombosis, pulmonary or cardiac complications (very rare) can occur or precipitate in patients with multiple pre-existing diseases diagnosed or undiagnosed by routinely available standard tests or if multiple procedure (in other parts of the body like abdomen/thigh/buttocks) are combined in one sitting. Kindly do not force your surgeon for combining multiple procedures in different areas of the body in one sitting which is never recommended by Dr Amit Agarwal and might turn a very safe procedure to an unsafe one.
  • Possibility of revision surgery if the results do not meet your expectations.

Breast feeding may be still possible, but it cannot be guaranteed because this surgery removes many milk ducts that lead to nipple. It is suggested that you should undergo the surgery after your family is complete. Pregnancy can loosen your breasts to some extent and breast reduction also involves breast lift of some degree, so it will be better to undergo the procedure after pregnancy.

Do I need to quit smoking after the surgery?

Quitting smoking is a perfect solution even without surgery as it involves many health risks, but it's totally up to your wish that you want to quit or not. But you should surely stop smoking at least 2 weeks before the surgery.

After how much time period can I resume my job?

You can resume work just after a week of the surgery, but you surely have to wear pressure garment for best results and proper healing and follow your doctor's instructions.
