Hemangioma is a benign tumor of endothelial lining of the blood vessels of different parts of the body.

They are non-cancerous so nothing to worry about the life expectancy of patients.

Hemangioma is the most common benign tumor of infancy period of children with the incidence of 4% to 10% by the age of 1 year. It is more frequent in premature low birth weight babies i.e., 23%.They don't cause problems in most babies. However, some hemangiomas may bleed or ulcerate. This may be painful also.

60% of hemangiomas occurs in head and neck region, 25% occur in trunk, 15% occur in extremity.

80% of times, haemangioma manifests as single tumor. These tumors usually develop within the first few weeks of life but these usually resolve spontaneously by the age of 10 years. Depending on their size and location, they may be disfiguring also.

Classification of Hemangiomas

  1. Congenital Hemangioma - Less common, present in 30% of cases, present at the time of birth, grows proportionately with the child's growth, are either rapidly, self-involuting (within 12 to 18 months) meaning they will shrink themselves or do not involute at all.
  2. Infantile hemangiomas - more common, present in 70% of cases, develop after 2 to 8 weeks of birth, grow rapidly for 6 to 12 months and slowly involute/disappear eventually upto 10 years of age.

Diagnosis, outlook and future course of Hemangiomas

Diagnosis is usually by history and physical examination. Sometimes a Ultrasound, CT Scan and MRI scan is done to see its extent and nature.

Size of hemangioma and sex of patient do not affect the speed and completeness of resolution.

Site of hemangioma has minimal effect on final result.

Multiple hemangiomas do not necessarily resolve at the same time and speed. Prescence of ulceration has no prognostic significance except for appearance of scar.

Treatment of Hemangioma

A single, small hemangioma usually requires no treatment. It'll likely go away on its own. Total involution occurs in 50% of hemangiomas by 5 years, in 70% by 7 years, and in 90% by 9 years.

However, some cases may require treatment, such as skin hemangiomas that develop ulcerations or bleeding, or are in specific areas on the face such as the lip, near eyes.

Medicines

  1. Propanolol : is the first line of drug. It is used for its effect on ACE enzyme expressed on immature capillaries of growing hemangioma. It induces apoptosis and fat development.
  2. Steroids : arrest growth of hemangioma but will not cause regression.
  3. Sclerotherapy : injection of sclerosing agent into the Hemangioma.

Pulsed dye and Nd : YAG lasers can be used in treatment of superficial heamgiomas, few ulcerated hemangiomas and removing residual color from regressed hemangiomas.

Surgery: Hemangiomas within the body may require surgery if they grow too large or cause pain. Surgery is typically delayed until the child is of school age and beginning to experience psychological consequences. Surgery is more commonly done in hemangiomas in adolescent and teenage years after they've regressed and left a residual scar or deformity over the region.

Surgery is preferably avoided in children with hemangiomas as they can cause more severe deformity after surgery. Emergency surgery is performed even in infancy period in babies if lesion threatens important structures or function like-

  1. Visual obstruction
  2. Airway obstruction
  3. Ear canal obstruction

Surgery of choice is Excision of lesion with or without flap cover and/or skin graft. Post op recovery is rapid and would require 3-4 days of hospitalization.
