Constriction ring syndrome or Amniotic band syndrome occurs when fibrous bands of the amniotic sac get tangled around a developing fetus.

Constriction ring syndrome or Amniotic band syndrome occurs when fibrous bands of the amniotic sac (the lining inside the uterus that contains a fetus) get tangled around a developing fetus.

Most of the time, the bands wrap around an arm, leg, fingers, or toes of the developing baby in the uterus. These bands can cause serious constrictions, acting like a tight rubber band around the body part at birth. Deeper bands can cause serious swelling of the body part after the band. These bands can cut off the blood flow, or do not allow the body part to grow properly. It can lead to amputation of the part also in severe cases.

Most children with constriction band syndrome will have bands on more than one part of the body.

Hallmark of the constriction ring syndrome is that the anatomy before the level of constriction or amputation is normal.

Classification

  1. Simple constriction rings- partial & circumferential
  2. Constriction ring with distal deformity, with or without concomitant lymphedema
  3. Acrosyndactyly
  4. Amputations

Treatment of Constriction Ring Syndrome

If the child needs surgery, it is probably done when they are 1 year old or older. But if the constriction is limiting the blood flow, they might need emergency surgery.

  • Under tourniquet control, constriction band must be dissected very carefully to avoid damaging the underlying neurovascular structures.
  • When constriction band is circumferential, treatment is two staged correction approach. At the first operation, ½ of the band is excised and the other ½ is excised in next stage which is after 3 to 6 months. Closure is done with Triangular flaps, Z–plasty or W-plasty.
  • Lymphedema when present, subsides within few weeks after the surgery
  • Usually, immobilization of the part is required till sutures are mature.
  • Patient is allowed to move the limb freely; no physiotherapy is required mostly.
