Gynecomastia is a condition of enlarged or overdeveloped breasts in men which can occur at any age. This condition can occur due to hormonal changes, obesity, can be hereditary ormay occur afterconsumption of few drugs/medicines. Gynecomastia is generally painless and feels like a rubbery mass below the surface of the nipple and can be observed either on one side or both the sides. Although Gynecomastia is painless, sometimes tenderness or sensitivity can be observed in the affected region.

Gynecomastia is broadly classified into two types:

  • Primary Gynecomastia appears during puberty and is found in males in the age group of 13-17 years. This generally happens due to hormonal imbalance when an increase in the hormone oestrogen over the hormone testosterone can be observed.
  • Secondary Gynecomastia appears in males of any age group and is generally associated with weight gain. This is often also called as pseudogynecomastia.

Preparations to be done for the surgery

  1. Consumption of Herbal supplements and anti-inflammatory drugs should be avoided as they can increase bleeding.
  2. Stop smoking at least two weeks prior to the surgery.
  3. Get the tests done which are prescribed by Dr Amit Agarwal.
  4. All pre surgical work up investigations should be shown to the operating surgeon and the anaesthetist, and we will evaluate your fitness for the surgery. You will be given date for the surgery only after you are declared fit for the procedure.
  5. Compression garment measurement should be given for post-surgical use.
  6. Admission will be done on the same day at the centre.
  7. Written consent form will be signed explaining the procedure after the admission.
  8. Meeting with the anaesthetist for Pre-Anaesthetic check-up before the surgery.
  9. Nil per oral for minimum of 5-6 hours before the planned procedure

Gynecomastia procedure includes following steps-

  • Anaesthesia: The surgery is usually performed under general anaesthesia except in very few cases where local anaesthesia is advised
  • Liposuction: The procedure involves removal of the excess fat and glandular tissues by liposuction technique. Tumescent anaesthesia solution is first infused in the fat tissues under the skin ofthe pre marked area of planned surgery to reduce bleeding, pain and trauma to the tissues.In cases where gynecomastia is primarily the result of excess fatty tissue, liposuction techniques alone may be used. This requires insertion of a cannula, a thin hollow tube, through one small incision below the chest level. The cannula is moved back and forth in a controlled motion to loosen the excess fat, which is then removed from the body by high pressure vacuum suction.
  • Gynaecomastia

  • Trans Nipple or Areolar Excision: If after liposuction, remaining glandular tissue is still prominently present or excess skin requires removal, excision technique is used to give a more natural male contoured chest. Dr Amit Agarwal prefers to use incision over the nipple areafor resection of the gland so that no scar is visible over the chest. In men with highly glandular breasts and very less fat, not amenable to liposuction or for excess skin removal, rarely incision is given on the areolar region for gland and skin excision. Any nipple areolar deformity can also be simultaneously corrected.

Recovery after the surgery

After the surgery, you are made to wear a compression garment to reduce swelling and to support your chest. You will be made to walk on the same or next day of the surgery. You will be discharged on second day of the surgery.

  • Single suture will be there mostly below the chest region which will be dissolvable and do not require to be taken off.
  • There can be swelling and bruising of the surgical area which usually subsides in 2 to 3 weeks.
  • In the days following surgery, there will be pain which can be subsided by the pain killers given by Dr Amit Agarwal.
  • Soreness might last for few weeks.
  • You will be made to walk 6 hours post-surgery or the very following day, depending on your will and wish.
  • You can start exercising after three weeks post-surgery.
  • Pressure garment is a must wear for the next 4 to 6 months without gap or fail for almost 24 hours a day.
  • You can return to work depending on your will power and the type of work required between 3-6 days.
  • You can return to your normal lifestyle after a week of surgery.

Risk and complications post-surgery

The complications and after maths of the surgery vary from person to person and their physical well-being. Before going for the surgery, you should always understand the risks and potential complications related to surgery. Gynecomastia surgery usually gives desired results but there can be some risks involved which include:

  • Rare risks related to General anaesthesia. Anaesthesia is very safe in present times and with best machines and a qualified anaesthetist, chances of encountering any complication is less than 0.1 percent in a healthy individual.   • Seroma formation or fluid accumulation can occur which sometimes persists for a week.

  • Irregular contours, which can be corrected by the use of the pressure garment
  • Poor wound healing because of poor blood supply (mostly seen in smokers, people with high BMI, diabetics, hypothyroidism and in people with multiple diseases).
  • Numbness or altered skin or nipple sensation can persist for 3-6 months.
  • Persistent pain which subsides with time and slight medications.
  • Prolonged swelling and skin discoloration which subsides with time.
  • Rare event of Deep vein thrombosis, pulmonary or cardiac complications can occur or precipitate in patients with multiple pre-existing diseases diagnosed or undiagnosed by routinely available standard tests and if multiple procedures are done in the same sitting.
  • Possibility of revision surgery if the results don't meet your desired expectations.
